Mazie's Mission Adoption Application
Thank you so much for your interest in one of the Mazie's Mission's rescues! We feel confident that your next family member is waiting for you. Please take the time to answer the following questions as it will help us pair you with the appropriate pet. Filling out this application does not guarantee that you will be approved to adopt one of our pets. We reserve the right to refuse any application.
Qualifications to adopt a pet:
1) Must be 21 years of age.
2) Have a valid driver's license with your current address.
3) Have the consent of all adults living in the home.
4) Have the time and money to invest in a lifelong commitment of adopting a pet.
